Online Service Requests
Many technical services can be requested online via our Service Request Forms below. If you need general assistance
with a computer problem, you can use our all-purpose online form to Report
a Technical Problem. However, for general computer problems we do strongly prefer that you call us at extension 5-Help
or use CUbhis eSupport for immediate troubleshooting.
Form Requirements:
An active UNI account is required to submit an online service request.
You must use either Internet Explorer v6, Netscape v7, or Firefox v1.5 to submit any of these forms. Older versions of web browsers
will not work properly.
Before accessing certain forms, you may first be directed to important policies regarding the request. Please read these
closely to avoid delays or problems with your request.
All of the forms have required fields, denoted by a red asterisk. You must complete all required fields or the form will not be
submitted.
Many of the forms have fields that require special formatting. These will have an example to the right of the field that
you must follow.
Many require approval by a Department Administrator or Chair. An automated email will be sent to them upon form submission.
We must recieve an email response indicating approval before we can continue to work on the request.
After successfully submitting your request, you will receive a ticket number for tracking purposes. You can then contact

Review your ticket status online
link to check for updates.
General Requests
Software Installation - if you'd like a
pre-licensed program installed immediately, please connect to eSupport from the PC that requires
the installation.
Network Connectivity Requests
If requesting a new IP address and network port activation, both request forms must be filled out and submitted separately.
Advanced Technical Requests
These requests require approval by a Department Chair, Director or above for approval, as well as a high level of technical knowledge about the
system and applications for which you are requesting resources.
